- Release Date: December 25th, 2002.

General Information / Changes
- Final DanceDanceRevolution arcade game to run on BEMANI System Digital 573 Hardware.
- First DanceDanceRevolution game to introduce the MARVELOUS timing judgment, which is ranked higher than a PERFECT. It is only used in courses.
- In normal gameplay, the arrow flash will be white instead of yellow if the arrow is hit within the MARVELOUS window.
- First Japanese arcade DanceDanceRevolution game to include the BEGINNER difficulty, first seen in Dancing Stage EuroMIX. Like in that title, dancers appear on the screen giving instructions to players on how to step in rhythm. Background animations are turned off.
- A screen demonstrating the usage of Freeze Arrows is shown before the game begins when anything other than HEAVY is selected.
- The bitrate of the songs' audio has been reduced to 112 kbps due to disc space limitations.
- On the music select screen, the foot ratings for PARANOIA survivor MAX and The legend of MAX's charts rated Level 10 glow.
- NONSTOP Mode returns.
- The CHALLENGE MODE exclusive songs are now available as regular songs, most of them in the DDRMAX2 folder. (HYSTERIA 2001 appears in the beatmania folder.)
- Their banners and backgrounds are also re-colored from the original ones that are seen in DDRMAX2's Challenge Mode.
- If either player is playing on BEGINNER or LIGHT difficulty, 1st stage cannot be failed.
- First DanceDanceRevolution title to include pop'n music, pop'n stage, GuitarFreaks & DrumMania, KEYBOARDMANIA, ParaParaParadise, MAMBO A GO GO, and beatmania III songs.
- Only arcade DanceDanceRevolution game to sort beatmania, beatmania IIDX and beatmania III songs with separate labels.
- First arcade DanceDanceRevolution title to indicate a DanceDanceRevolution arcade original song's game of origin, instead of listing it as a KONAMI original like in previous games.
- However, all pre-EXTREME Dancemania licenses are given a generic Dancemania logo.
- Last arcade DanceDanceRevolution game featuring long-time designer RICE.C.
- Songs that made their DanceDanceRevolution debut in DanceDanceRevolution 2ndMIX CLUB VERSiON 1 and 2 that did not have unique Double HEAVY charts got new ones.
- RANDOM selection has been added. Unlike ROULETTE, RANDOM selects a song instantly.
- When RANDOM is selected, a sample of selected song will be heard in the options screen. This is the only arcade DanceDanceRevolution game to do this.
- First DanceDanceRevolution game to feature the modifiers BRAKE and WAVE, as well as x0.25 and x0.5 speed modifiers. These only appear on certain ONI courses.
- Last DanceDanceRevolution game to show the DANGER message/background video when the life gauge is low.
- Also the last game where the life gauge animates based on the scrolling speed of the chart currently being played.
- Last DanceDanceRevolution game to draw arrows in white as they fade in or fade out when using the HIDDEN or SUDDEN modifier.
- Last DanceDanceRevolution game to use the sudden death life gauge in ONE MORE EXTRA STAGE.
- Last Japanese arcade DanceDanceRevolution game for four years, until the release of DanceDanceRevolution SuperNOVA in 2006.

- Color labeling on the Music Select screen is as follows:
- green: New Konami original songs and Dancemania licenses.
- purple: Konami original songs and Dancemania licenses that made their debut outside of the main arcade series (CLUB mix, DDR Solo series, console DDR releases) .
- blue: Older Konami originals and Dancemania licenses from the main arcade series.
- yellow: BEMANI crossovers, both new and returning.

In EXTREME, the use of APPEND codes by the arcade operator was restored and the point system added in DDRMAX2 was removed. Similar to how previous APPEND codes worked, each code also unlock the preceding PASSes. For example, the 3rd PASS will unlock the content included in the 1st PASS up to the 3rd PASS. To unlock everything at once, use the 7th PASS (FINAL) code.

FINAL STAGE
The following songs are only available during the FINAL STAGE. PARANOIA survivor MAX appears instead of the original PARANOIA survivor by clearing PARANOiA Rebirth's HEAVY chart with a AA grade or higher during the 1st STAGE, and then clearing PARANOIA EVOLUTION's HEAVY chart with a AA grade or higher during the 2nd STAGE.
The 7th PASS unlocks these songs along with all of its charts to be playable outside of the FINAL STAGE.

EXTRA STAGE
To obtain the EXTRA STAGE, you must AA a HEAVY or CHALLENGE chart in the FINAL STAGE. Afterwards you will unlock the EXTRA STAGE, which will force 1.5x Speed, Reverse, and a "Pressure" Life Gauge on you, regardless of what song you play. You will also be forced onto the HEAVY chart of a song (if a song has an CHALLENGE chart, it will force that on instead).
The legend of MAX can only be played during EXTRA STAGE on HEAVY. However, the 7th PASS unlocks the song along with all of its charts to be playable outside of the EXTRA STAGE.

ONE MORE EXTRA STAGE
To access the ONE MORE EXTRA STAGE, you must AA The legend of MAX as your EXTRA STAGE. You will then start playing Dance Dance Revolution on CHALLENGE difficulty with 3x Speed, Reverse, and a life gauge set to RISKY (one Good/Boo/Miss/N.G., and the song ends).
Similar to the EXTRA STAGE song, the 7th PASS unlock the song and all of its charts to be playable outside of the ONE MORE EXTRA STAGE.

Unlock Infomation
Like DDRMAX2, EXTREME has multiple unlock codes available, however there is no way unlock songs via game play.
- Press Test button to enter the Service Menu.
- Select "Game Options".
- Hold 1P down panel and Service button, and then press 1P start button. The "secret" menu will be revealed.
- Select "Append".
- Input the password in order as shown below. Use 1P left and right selection buttons to select spaces, and 1P start button to change characters.
- After entering the password, hold 1P left and right selection buttons, and then press 1P start button to confirm the password.
- Select "Exit" to game options menu. Do not select "Initialize".
